THIS IS NOT CARE.DATA.

SUMMARY CARE RECORD IS AN ELECTRONIC HEALTH RECORD

THAT PROVIDES HEALTHCARE STAFF WITH RAPID ACCESS TO ESSENTIAL

INFORMATION ABOUT A PATIENT IN ORDER TO PROVIDE THEM WITH DIRECT

CARE AND TREATMENT.

http://systems.hscic.gov.uk/scr.  This is the link on the HSCIC Website which should give you all the information you need relating to SCR.

http://systems.hscic.gov.uk/scr/staff/newpatients. This will enable you to print off copies of the opt out form, if you wish to.
