Stroke Sufferers – can you help with your feedback?

Our CCG is looking to get feedback from any patient (or their carer) who has suffered a stroke and have produced a ‘stroke review document’ – please see further information below.

The simplest way for respondents to provide the information requested is by logging into the online version which can be found via the link below.

Redesigning stroke care

The NHS across Sussex is reviewing stroke services throughout the region and we would value your feedback on how services are currently caring for people locally. You could help shape the future of stroke care in Sussex by sparing a few minutes to complete a short questionnaire.

Carer questionnaires – http://mipro.li/j2NLn7s If you have experienced more than one stroke or TIA (Transient Ischaemic Attack or mini stroke) please refer to your most recent experience. There is also a questionnaire for carers of someone who has had stroke.
